At UNIFIED LAB SERVICES, we want to ensure that we provide you with the most exceptional experience both online and in person. To do so, here is our list of policies and procedures to ensure that your experience with our company and staff is smooth and hassle-free.

Cancellation: Please provide our office with a 24-hour notice in the event that you need to reschedule your appointment. A message can always be left with the answering service to avoid a cancellation fee being charged. If it is not at least 24 hours before your appointment, you will be charged a $50 reschedule fee.

Behavior: We like to make sure that our customers are able to enjoy their visit in a fast and friendly manner. To do so with perfection, running, playing, using profanity, or destroying equipment or furnishings is not allowed. If you do not comply, you may be asked to leave, and further action may be taken if necessary.

Late: If you are more than 15 minutes late, you will only be seen if we have availability for that day. If there is no open availability, the appointment may be canceled and or rescheduled and may be subject to a $50 fee.

Refund: There is NO refund at this time.

No Call or No Show: A “no call – no show” will result in you having to pay for another


Travel Fee : There is a Travel Free access for all clients. 

0-25 miles - $25 Flat Fee

26- 50 miles - $50 Flat Fee

50-100 miles - $125 Flat Fee

Weapons: We are a weapon-free establishment, and weapons and firearms of any kind are prohibited in our office.
